The challenges facing the adult social care sector are significant. We believe that with the right staff, leadership and vision, we can meet these challenges and work with residents and partners to come up with creative solutions – always keeping residents at the centre of their care.

With an ability to think strategically, challenge the status quo and negotiate effectively you will also possess an insight into what works operationally and of what good commissioning looks like.

You will have responsibility for  ensuring that placements continue to meet the assessed needs of individuals placed and that they meet expectations in terms of quality of service and cost effectiveness. This will be achieved by 

Progression:

Identifying our service users’ potential for Progression/Moving On and creating SMART targets to help them achieve their goals through:

- Outcomes-focused practice.

- Progression planning.

- Improved ways of working (including provider engagement)

Moving On:

Creating a logistics-focused plans that support service users to move to their future settings as smoothly as possible through:

- Streamlined matching process.

- Improved tracking of barriers to progress.

Commissioning:

Supporting Commissioning to understand projected cohort shift and moves between settings through:

- Forecasting demand vs. capacity for settings.

- Identifying opportunities within the Commissioning landscape.

- Improving the flow of information from operations to Commissioning.

To further ensure that all commissioned provision is meeting required contract standards and outcomes.

To support and contribute to the delivery of an annual commissioning delivery plan and to support ongoing commissioning and market development..

We are seeking to recruit two key posts to our newly established Commissioning Reviews Team. The Senior Practitioner role will sit within the Adults Integrated Commissioning service. You will work with our local care market, social work teams  and our commissioning leads to ensure that services are delivering optimal outcomes for all service users across a range of adult client groups.

You will be expected to demonstrate an understanding of the importance of commissioning the right support at the right time to enable people to move toward independence and live in the least restrictive way possible.  You will encourage informed positive risk taking in services to ensure that people can progress, are maximising their personal strengths and are embedded in their communities, at all times.
