Hospital Discharge Lead PO3
An exciting opportunity has arisen within our Homeless & Prevention Service to work in partnership with the integrated hospital discharge teams across both Whipps Cross and Thorpe Coombe Hospital.
To develop and Adult Social Care Discharge Team, Mental Health teams and Housing Solutions on borough wide hospital discharge and all related legislative and service issues. To successfully manage the transition from hospital to an appropriate home wherever possible, including designing, agreeing and oversight of pathways to facilitate discharge between services. To be responsible for data gathering and analysis to inform service development and to provide monitoring information on outcomes to Housing and ASC.
The post holder will deliver specialist support/advice to the integrated hospital discharge teams where a patient’s discharge may be delayed as a result of housing related matters. The post holder will successfully manage the transition from hospital to an appropriate home wherever possible, including designing, agreeing and oversight of pathways to facilitate discharge between services. To be responsible for data gathering and analysis to inform service development and to provide monitoring information on outcomes to relevant partner agencies.
MAIN DUITES AND RESPONSIBILITIES

Receive all referrals throughout the day and record on the Council’s housing system. -
Produce reports on patient’s length of stay in hospital and those who are ready for discharge, with their agreed pathway, to include confirmed care packages -
Email all documents to relevant departments - Liaise with wards, Adult Social Care, Housing Services, Support Services, Patients, their families and Advocates.

Receive enquiries from various departments and respond to them appropriately. - Where required to hold a caseload of clients and assess their housing needs in line with the Homelessness Reduction Act 2017.
Role is for a Fixed Term 12 months
