About the Team:
We are looking for a qualified Social Worker for our Front Door Community based service: Information, Contact and Assessment Service ( iCAS).
iCAS is at the forefront of Adult Social Care’s service to Lambeth residents delivering a comprehensive assessment service to vulnerable adults. We are looking for a competent and compassionate social worker who can work at a pace. In order to maximise resources and achieve customer focused outcomes, you will need to have excellent communication and planning skills and be committed to partnership working across statutory and third sector organisations.
About the Role:
Key Responsibilities are:
- The team carry out assessments with adults over 18 years of age, with a range of physical health disabilities and vulnerabilities, so experience with these client groups is essential – as well as having a robust understanding of the Safeguarding procedures.
- The primary feature of work undertaken by staff is to deliver strength based preventative support, where required.
- The team undertake Assessments and support with signposting, primarily to adults who have approached the council for the first time.
- Social workers carry out a variety of responsibilities, including referring people to reablement where they are identified as potentially benefitting from maximising their everyday living skills and they consider the provision of services via Direct Payments.
- The team work closely alongside our OT and Age UK colleagues.
- iCAS is a fast-paced team with a committed and enthused workforce who strive to achieve positive outcomes for our residents.
- You will work closely with other teams in the borough and engage wider agencies to support the person.
- You’ll need to adapt to a demanding and changing environment and have a commitment to partnership and collaborative working.
- The team support hybrid working, the requirement is to be on site 2 days a week with the option to work 3 days at home.
