What You’ll Be Doing
The Mental Health Pathway Referrals Co-ordinator is a key role which coordinates referrals for individuals with severe and enduring mental health needs, ensuring they are supported as they move into and through specialist supported housing options. This operational and strategic position requires you to engage with various internal and external stakeholders, including mental health professionals, commissioners, and housing providers. Your responsibilities include improving referral processes, managing a caseload, addressing barriers to housing, and ensuring compliance with relevant legislation such as the Homelessness Reduction Act. You will also play an important role in staff training and contribute to the development of policies and procedures.

We are looking for an enthusiastic individual who is committed to providing relational and supportive services that achieve the best outcomes for residents. You will have a passion for housing equality and ensuring that all residents, particularly those with mental health challenges, have access to safe, stable, and supportive accommodation. Proactive and solution-focused, you will be able to respond to complex cases while balancing the pressures from multiple stakeholders. Strong communication and collaboration skills are essential, as is a clear understanding of the barriers faced by vulnerable clients. Your ability to manage processes and ensure effective outcomes for clients will be central to your success in this role.
