About the role:

As an Adult Early Help Link Worker you will support residents with emerging needs and independent living and wellbeing needs, as outlined in the Adults Threshold Guidance, to identify and achieve self-defined wellbeing goals. Act as a single, local point of contact in order to provide advice, information, and support in the community to individuals and of all ages and across service types and their families including carers, who may be facing difficult life situations and who may be disabled, older or have mental health issues.

Adult Early Help Link Workers work within a strength-based practice model, ensuring relationships are built with, compassion, responsibility, collaboration, curiosity, creativity and is focused on outcomes that enable people to be safe, well, resilient and independent.

Adult Early Help Link Workers work closely with Adult Social Care teams and voluntary and community sector staff within a broader wellbeing network. Link Workers will work within Welcome Hubs, community settings and with a range of other professionals.

Key Responsibilities:

As an Adult Early Help Link Worker you will have experience of working alongside and supporting individuals and families from a variety of backgrounds and with varying abilities, strengths and needs. You will have have the ability to build and maintain effective working relationships with individuals with emerging needs, alongside supporting people to access accurate, timely and relevant information and assist individuals, families and communities to access information through a variety of means.

As a Link Worker you are able to identify and respond to potential safeguarding situations, maintaining effective longer-term support. You will have the ability and experience to identify risk and assess/manage risk when working with individuals.

You have the ability to maintain and develop effective working partnerships and relationships with key referral services such as Adult Social Care.

Qualifications and Requirements:

Educated to degree standard or equivalent

Relevant qualification in Community Development, Education, Health or Social Care - A degree or post-graduate degree level or equivalent.

Considerable post qualification experience is required.

OR

Relevant degree qualification or equivalent or 5 years' experience in a similar role at a similar level. All certificates to be provided at interview for validation
