About the role:
We currently have a vacancy for a qualified social worker in the adult review team. We work with adults from 18 to 100 plus years who do not sit within specialist services.
All service users and carers are entitled to an annual review of their care and support needs which we are responsible for completing in a timely manner. We complete strength based reassessments looking at local support networks, use of technology or sign posting to other appropriate services. You will be expected to amend support plans to reflect the needs of the person and complete any tasks that come from completing the review. This will include identifying informal carers and completing carers assessments. Reassessments are mainly completed in person so you need to be physically able to complete this task. You will need a good understanding of safeguarding processes. You will need to be able to work on your own and make appropriate use of supervision and the management support available. Key Responsibilities:
You will work with adults aged 18 and over, to complete individuals' annual reassessments and reviews with them. It is a fast-paced role and there is an expectation that all pieces of work with people will be completed timely. As well as annual reassessments the team do identify safeguarding issues and complete the safeguarding process. We provide a strength-based person-centred approach to service users and informal carers, to enable then to enhance their well being, independence and minimize risks. Your day-to-day responsibilities may include:
- Completing annual reviews and reassessments for service users and carers.
- Complete any work generated by the reassessment or review.
- Recording information in a timely manner
- Completing Safeguarding process
- Work collaboratively with internal partners as well as our Health Partners, Statutory and Non-Statutory organisations to secure a holistic approach to Assessment, Safeguarding and Care Planning.
Qualifications and Requirements:
Educated to degree standard or equivalent
Relevant professional qualification (BA Social Work of equivalent)
Social Work England registration
Have good computer skills: Microsoft Office, Mosaic, Oracle, Imosphere, Teams
You need to be able to physically able to complete visits in the community which may involve stairs or travelling
