 About the role

We are seeking a motivated and compassionate individual to join our team as a Rough Sleeping HOST Case Officer. In this essential role, you will be at the forefront of our efforts to work with rough sleepers and provide vital housing support to vulnerable individuals in Tower Hamlets. Your dedication will directly contribute to helping individuals transition out of rough sleeping situations and into stable housing.

We are seeking a motivated and compassionate individual to join our team as a Rough Sleeping HOST Case Officer. In this essential role, you will be responsible for the statutory homelessness assessment of rough sleepers providing support, developing, with them, the personalised Housing Plan. You will work with them to implement interventions to help them transition from the streets into stable housing.

As a Rough Sleeping HOST Case Officer, you will engage in outreach activities to identify and connect with rough sleepers in our local area.

You will work closely with the Street Outreach, the Pathways, housing providers, healthcare professionals, and support agencies to coordinate holistic support for rough sleepers.

The Person

We are looking for a candidate with previous experience in statutory homelessness knowledge and case working skills under the Housing Act, or homelessness support.

We are looking for a candidate with previous experience in outreach work, social healthcare coordination, with a specific focus on homelessness prevention and intervention support, specifically with expertise in statutory homelessness procedures and the Housing Act.

Strong communication and interpersonal skills are vital for building trust with rough sleepers and collaborating and ability to engage effectively with stakeholders within multidisciplinary teams to build positive relationships. A compassionate and non-judgmental approach, combined with the ability to work independently and as part of a team, will be essential in this challenging yet rewarding position.

If you have strong problem-solving abilities to assess complex needs and develop tailored housing solutions with exceptional organisational skills to manage caseloads and coordinate discharge processes efficiently. If you are confident, experienced, and are an adaptable self-starter with the ability to work proactively on your own initiative, then you may be the one we are looking for.
