Our Transfer of Care Team is seeking a Weekend Social Worker (covering Friday to Tuesday) for our busy team covering King's College and St. Thomas' Hospitals.

About the Team

The team provides support for adult service users requiring assessment and provision of services to support safe and timely discharges from the hospital.

Our social workers work closely with the hospitals' multi-disciplinary teams to undertake a range of activities to facilitate discharges within the Discharge to Assess framework, including assessing for and arranging nursing and residential placements, as well as generic packages of care.

The team works with a high number of service users with cognitive impairments; therefore, social workers regularly complete Mental Capacity Assessments and Best Interest Meetings/decisions. The team also receives Safeguarding referrals from the hospitals and carries out safeguarding enquiries as required.

We pride ourselves on offering regular supervision and support, nurturing individual skills and potential for growth within our team. Exciting opportunities are available to specialise and participate in Best Interest Assessor and Practice Educator courses, enabling you to further develop your skillset.

About the Role

As a Weekend Social Worker, you'll be at the forefront of our efforts, working from Friday to Tuesday. Your responsibilities will include conducting Care Act Assessments, Mental Capacity Assessments, and promoting strengths based, person centred practice.

You'll thrive in a fast-paced, multi-disciplinary environment, making key decisions and providing exceptional communication to service users, families and carers.

About You

We're seeking a resilient, confident and enthusiastic social worker who upholds the highest professional standards. To be successful in this role, you will:

  • Be a skilled decision-maker and communicator, with a passion for meeting the needs of the local community.
  • Have excellent report writing skills.
  • Have a strong understanding of safeguarding processes and the ability to work under pressure while problem-solving creatively.
  • Have a degree (or equivalent) in social work, have completed your ASYE year, and be registered with Social Work England.
