An opportunity has arisen for a Social Worker to join the Changing Futures Sussex Programme within Brighton & Hove City Council (Health & Adult Social Care  - HASC). In Brighton and Hove Adult Social Care we passionately believe that putting relationship-based practice at the centre of everything we do enables us to provide consistent and quality support to citizens within the city so that they can lead independent and fulfilled lives. About the role

Changing Futures Sussex Programme is one of 15 national Programmes bringing together public and community sector partners to improve the way local systems and services work for adults experiencing multiple complex needs (MCN).

The Programme aims to create an environment where individuals experiencing multiple complex needs, who repeatedly fall through the net of service provision, receive flexible, trauma informed, person-centred support. The Programme aims to enable positive changes in services; to make them better connected, and easier to access. Changing Futures Sussex works in partnership and is embedded across the three Local Authority areas: East Sussex, West Sussex, and Brighton and Hove.

Social Workers within Changing Futures will be part of a multi-disciplinary team who will adopt a curious, creative and collaborate approach to service provision. They will be responsible for discharging the local authority’s statutory duties under the Care Act 2014 and Mental Capacity Act 2005. We are looking for innovative thinkers and those that truly believe and practice a trauma informed approach. The team includes Social Workers, Homeless Prevention Officers, Community support workers, a Domestic Violence and Abuse Worker, a Recovery Coordinator, Mental Health Nurse as well as Peer Support workers with lived experience.   The team is part of the wider Changing Futures Sussex Systems Change priorities to identify blocks and barriers in the system, recognising best practice and flexible working to overcome the challenges facing people with multiple complex needs.

Multiple disadvantage or multiple complex needs is defined by the national Changing Futures Programme as people who experience three or more of: homelessness, current or historical offending, substance misuse, domestic abuse, and mental ill health.

The role covers service delivery across Brighton & Hove.  Hybrid working arrangements are applicable to the role, this includes elements of office-based working, client visits and working from home.
