Peabody has an exciting new opportunity for a Community Support Worker. You will be involved in the day to day operations of a new support hub commissioned by Tower Hamlets at the Sundial Centre E2.

You will be working with a range of external and internal partners, as well as providing support to vulnerable adults with a range of needs including mental health needs, mild learning disabilities, age related conditions and physical disabilities. You will be the main point of support from referral to exiting the service.  You will be committed to encouraging customers to live independently and enhance their quality of life.

You will be responsible for completing support plans and risk/need assessments, as well as assessing new referrals to the service. You will need to have a good understanding of mental health, some experience of providing support to this designated customer group in a professional role and demonstrate the values that you will need to work with this sometimes challenging client group. You will have the confidence once inducted to work on your own initiative.
