About the role
This is an exciting time to be part of the Housing Options Service and the Council’s major new Homelessness Transformation Programme. You will be part of a team working with the Council's commissioned hostel services supporting clients that have various support needs. Your main role will be to coordinate move-on pathway for rough sleepers, many of whom will have substance misuse issues, out of hostel and temporary accommodation into treatment and/or longer-term accommodation.
You will be part of a team promoting substance misuse reduction and recovery, stabilise, and improve health, reduce the need for acute statutory interventions. You will support rough sleepers to access employment, training, and education.
We are looking for someone who will assist the Hostel Pathway Coordinator in identifying the barriers, whether policy, resource, procedural, organisational, or statutory and find solutions with accommodation managers, commissioners, colleagues in Housing Options RESET, and other stakeholders.
Your responsibilities will include, amongst others, drafting SLAs, procedures, and protocols to facilitate the smooth and efficient running of the Pathway. Also optimising the number of rough sleepers gaining access and support from the Pathway
A confident, experienced, and adaptable self-starter with the ability to work proactively on your own initiative, you will be expected to work collaboratively with all partner agencies to provide an optimum service that provides evidenced outcomes.
You will facilitate the Rough Sleeping Drug and Alcohol Treatment Grant (RSDATG) Project Group meeting in line with the Terms of Reference. In addition, you will manage an active caseload of rough sleepers with substance misuse issues including Target Thousand Rough Sleeping clients, to ensure that their progress is monitored, they engage with treatment services and move on into longer term accommodation. This will include hosting case conferences, one to one meeting with clients and close collaboration with other services
