About the role
The Children with Disabilities Team is a specialist Social Work team providing support, advice, assessments and access to specialist services to children and young people with severe disabilities and their parents/carers resident within the London Borough of Hillingdon
The role of a FSW is crucial in reviewing and monitoring care packages and supporting disabled children and their families under a child in need plan. Children who are in receipt of additional services and or care packages to minimise the impact of the significant needs of the child on the day-to-day living arrangements of the child and their family/carers.
Family Support Worker will be holding a manageable case load and will be responsible to maintain regular contact with families and involved professionals. Family Support Worker will be expected to carry out home visits to see children in their home environment.
