The ASCIDH plays a vital role in the management of complex discharges from hospital settings working with Social Work, OT, Nurses staff and other professionals to provide a holistic and person centred assessment and care management approach. In line with the Delayed transfer of care policy and the 2014 Care Act.
Successful applicants will be expected to hold and manage a varied caseload, the post holder will be required to carry out Care Act assessments, CHC Checklist and DSTs, complete support planning, develop long term support arrangements, undertake Mental Capacity Assessments, Deprivation of Liberty Safeguards and Best Interest Assessments and will be required to undertake reviews for customers both during and post hospital admissions.
As a team of Social Workers and Social Care Officers (in the Integrated Discharge Hub) we provide a frontline service to a wide range of customers over the age of 18 with a spectrum of different social and health care needs, including customers with physical impairment, sensory loss, learning disabilities, older adults and dementia, all of whom have presented to hospital. Successful candidates will ensure that the team analyse risks and facilitate timely discharges from hospital supporting customers and their families to develop innovative and creative strength-based support plans to customers following discharged from hospital.
About you:
We are looking for candidates who are registered Social Workers with the Social Work England and possess extensive post qualifying experience within adult social care.

- Able to undertake timely and effective assessments that evidences a strengths based approach
- Able to undertake timely and effective assessments, support plan and reviews post discharge under the Discharge to Assess (D2A)
- Committed to achieving safe and effective discharges from hospital
- Have good problem solving skills
- Are able to demonstrate your ability to prioritise your workload within a pressurised environment,
- Have excellent skills in identifying and managing risk
- Are dedicated to the building and maintaining effective professional relationships in line with the integration agenda.
You must have excellent working knowledge of relevant legislation in the Care Act, and social work methods, models and theories. You must have the ability to sustain a clear performance focus on achieving demanding goals to attain the best possible outcomes. You need to be able to collaborate constructively with internal and external partners to create the conditions for successful partnership working with our health colleagues to manage the demands of acute hospital settings.
