About the role

The post holder will be part of the The Carers Team with Adult Social Care and Health’s Access Point. Your focus will be on assessing the needs of the informal carer and joint working with the Carers Centre to begin them on their support journey towards greater wellbeing and resilience to maintain their caring role. You will provide creative and preventative interventions including Personal Budgets, daily living equipment and assistive technologies as part of wider social care assessments to support both the informal carer and the person they care for. You will provide practical support and identify appropriate referrals and interventions to promote or maintain independence and well-being. Management of risk and identification of timely interventions which can promote independence is key to this role.

Experience of working for Adult Social Care within the community is essential. An understanding of the needs of carers and the ‘strengths based’ approach to support is fundamental. An appreciation of the role of daily living equipment and technology is important but full training on the Enhanced Assessor role and Telecare interventions will be provided.

Our ethos here in Brighton & Hove is to put our clients at the centre of everything we do and we actively encourage staff within the team to work together with other colleagues across the wider Adult Social Care service so that we can better meet the needs of our clients.
