The role
We are looking for a dynamic, proactive, Assessment and Case Coordinator. You will have good attention to detail and accuracy as well as being able to see the ‘big picture’. An ability to juggle competing demands all with statutory timescales is a must so you will need to be resilient and organised.
You will be deeply committed to children and young people.
The main objectives for the post holder are to understand and synthesize reports into a document which can be easily understood and implemented by professionals. You must be a highly effective communicator with a good understanding of social care processes as well as the Children and Families Act 2014. This role will require travel within and outside of the Borough and London. You will support the statutory needs assessment process and ongoing monitoring of EHC Plans for children and young people who are Looked After, under a Child Protection Order, a Child in Need, subject to CETRS, or placed in out of Borough residential settings and those with complex needs who may be at risk of placement in hospital.
The ideal candidate will take lead responsibility for monitoring this cohorts’ outcomes and progress which will require close working across multi-agency teams, families, carers as well as securing the voice of the child or young person and ensure all statutory deadlines are met. You will take the lead for education which will require a good understanding of education across different phases and types from early years to post 16 and across mainstream and special, to ensure the continuum of a young person’s special educational needs are being met and that the Special Educational Provision they require is reviewed annually.
This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.

You must be a highly effective communicator who possesses the ability to interact with people at all levels including Parents, Carers, Young People, Head Teachers, SENDCos, Social Care and Health Practitioners and others as required. You will work across two teams with a primary focus on a particular group of identified children and young people. You will have knowledge of SEND law and its application with regard the Children and Families Act 2014 and the SEND Code of Practice 2015 as well as high levels of knowledge in relation to the Care Act 2014 and Social Care practice and processes. You will work under the direction of the Senior SEN Officers in collaboration with SEN colleagues, to fulfil your statutory duties, but also the legal department and across all allied services to plan and prepare for the Local Authority’s case in response to internal complaints, the Local Government Ombudsman or Special Educational Needs and Disability Tribunal with regard to decisions on provision/placements.
This post demands a high level of flexibility, a positive attitude and ability to adapt to changes due to service needs. The post holder will be expected to work in an open plan office environment in which ‘hot-desking’ and agile working practices are implemented and working at home as required or in agreement with the SEN Manager. The post holder may be expected to work at other sites, including in other boroughs and attend annual review meetings outside of the Borough and London.
