The Post

 

The Brent Community Learning Disability Service is a forward looking team which is co-located within the Kingswood Centre. The Brent Community Learning Disability Service is part of  the Learning Disability Service at Kingswood Centre, which has been rated Outstanding by CQC. The team works closely with residents, carers and Independent Providers to enable a person centred service to adults with Learning Disabilities. We all work in partnership to make a positive difference to the lives of adults with learning disability in our community.

 

We are looking for an experienced Social Worker who will hold a varied caseload, which they will manage. You will deliver high-quality care to the wider community. This will mean producing high-quality work in terms of the Care Act and associated legislation, assisting with challenging cases in terms of the Community Treatment Review process, and the ability to respond promptly to crises as well as hospital discharges.

 

Reporting to the Team Manager you will help reshape the way we deliver services to our residents in a COVID world. This will involve working to improve the life chances of those with Learning Disability through employment, accommodation, meaningful activities, as well as person-centred services, utilising best practice. You will have excellent communication skills. This will include producing high-quality support plans and working with families and networks against a backdrop of competing demands.

 

The Person

You will hold a Social Work qualification, be passionate about serving our residents, and have excellent communication skills and the creativity to find solutions that deliver excellent services our customers deserve. You will have post qualified experience working with people with Learning Disabilities, Mental Health or Autism.

 

This role would suit someone who has a passion for ensuring the delivery of high quality services and who enjoys managing and supporting people to achieve their potential. This role would suit an individual who is committed to improving the services for people with learning disabilities.

 

You will work in partnership with the management team to deliver on national and local performance framework. Ideally you will have experience in working with complex, multi-disciplinary cases involving mental health, voluntary organisations, forensic and NHS/CCG partners.
