Transforming Reablement for our residents

Two Occupational Therapist posts have been created in Brent’s Home First and Reablement Team as part of Brent Adult Social Care’s transformation programme and the wider development of the reablement service across Brent Adult Social Care with the aim of working with people to build their confidence and skills to achieve a jointly agreed optimum level of independence.

Brent Adult Social Care is committed to an organisational culture that invests and empowers its staff to take ownership and accountability to enable staff to provide excellent services to residents to help keep them safe in the community. Person centred services are aimed at maximising independence through building resilience and empowerment to enable people to manage their care and be independent.

The Post

This is an excellent opportunity to work in a progressive and Multi-disciplinary team comprising of Social Workers, Care Assessors and OT assistants providing Home First and Reablement to clients in their own homes. The successful candidates will have key roles in developing:

Improved pathways with the existing community social care and complex care  functions to increase referrals from community

Services for clients with Learning Disabilities and mental health needs to  support those groups being offered the opportunity to promote their independence through a reablement service.

Development and training with homecare providers to promote and facilitate clients with long term care packages to maintain their independence and autonomy.

Development and training with reablement providers who are commissioned to deliver reablement care packages.

Development and training for PAs who are already working directly with clients through Direct Payments to be able to deliver enabling care.

The Person

The successful candidates will ideally have:

post graduate experience in Social Care and or Intermediate Care

Experience of moving and handling in community based settings

Experience of assessing for major adaptations

Experience of working with Home Care providers and Reablement providers
