As a Social Worker within the Accommodation and Hospital Discharge Pathway service, you’ll provide advice and support to Mental Health Services.

The role

We are looking for a Social Worker who is keen, enthusiastic and passionate about Mental Health Services and delivering the best service to those in need.

Camden & Islington NHS Foundation Trust in partnership with Camden Council are currently looking for a Social Worker who can coordinate services in the delivery of agreed actions which will enable the timely discharge of service users from Inpatient beds once assessed as clinically suitable for discharge. The role will also provide advice and support to Mental Health Services in identifying the most suitable accommodation pathway for services.

This post works as a bridge between the in patients wards, bed management team and the relevant community teams (Care Coordinators). It will have a focus across both Camden and Islington boroughs and consider all service users regardless of clinical pathway they are under.

This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.

About you

This role requires you to be a qualified Social Worker and must be registered with Social Work England. Also, to be successful in this role you must be willing to show a commitment to undertake Post Qualifying Courses when relevant.

You will be a highly organised Social Worker, providing an updated list of progress and actions for each Borough, as well as being an excellent communicator, meeting with Bed Managers on bi-weekly basis and ward teams in relation to new admissions.

As the role will have primarily an inpatient focus the postholder will have a base at the Highgate Mental Health Centre (HMHC), however, is expected to spend time on both the HMHC and St. Pancras Hospital sites.
