The role

An exciting opportunity has arisen for an experienced, creative and flexible Occupational Therapist (OT) to join the Southwark Mental Health Reablement Team.

The Mental Health Reablement Team covers the varied and redeveloping borough of Southwark. We offer person centred social care assessment and support through the framework of the Care Act 2014; our aim is to reduce, prevent and delay the need for care provision.

We work with adults (18-65) who have a primary mental health diagnosis, are living independently and not receiving a package of care.

Service provision is up to 12 weeks, focusing on person-centred goals assisting people to maximise their independence by means of practising tasks, coaching them to consider alternative ways of doing tasks and building on their strengths and abilities.

We also promote and support engagement with the many services and activities available in Southwark. For those who may require ongoing support after Reablement we are able to put a personalised package of care in place.

About You:

You will hold a caseload of 10-12 clients comprised of both generic Reablement and OT specific cases which will usually require OT assessment and interventions guided by the Model of Human Occupation and provision of minor adaptation.
