Havering Children’s Services secured funding from the Department for Education’s Innovation Fund to launch the ‘Face to Face Pathways’ programme from September 2017.
Building on our systemic ‘Face to Face’ commitment to working more closely with families, this programme will put the Council at the forefront of innovation to improve the experience and life chances of children in care and those leaving care.
‘Face to Face Pathways’ will radically change how we engage with young people and deliver innovative solutions with them, not to them. We will ensure children and young people are at the centre of planning as their pathway is developed and transform our support to young people reaching adulthood, guaranteeing access to specialist services if needed.
At its heart, the Face to Face Pathways programme will be co-produced with children and young people. Through design, film-making and outreach, we will empower and enable young people to better determine their next steps and future.
Face to Face Pathways will:
- Begin pathway planning and transition to adulthood much sooner, at 14 years old up to 24
- Give ‘passported’ access to services, such as health, housing and education to care leavers, based on need rather than eligibility
- Offer care leavers more choice, more information and more support
- Provide specialist foster care through systemic training to enable young people with complex needs to stay close
- Stop children entering care by working intensively and systemically to enable families to stay together and build their own solutions
About The Job
- Co-ordinate care across service areas and the multi-agency network for young people aged 14-24 who are entitled to leaving care services.
- Work with the multi-agency network to passport young people to services based on their need and value added.
- Work in partnership with young people to co-produce pathway plans and services to improve the transition to adulthood.
- Work with partner agencies to remove barriers and improve continuity of care across services for children and adults.
- Champion, actively promote, lead on, facilitate and improve outcomes for one of the following areas: education, training and employment; general and sexual health; mental health; substance abuse; housing and homelessness; or immigration for children in care and care leavers aged 14-24.
- To champion, support and challenge all settings, and local authority services with regard to their contribution and the measurable outcomes for children looked after and young people aged 14-24.
- To be accountable for providing, in a timely fashion, all relevant information and statistical analysis with regard to the outcomes of children looked after and care leavers for a variety of audiences to include elected members and senior local authority managers.
- To support the Leaving Care Team Manager and other relevant service area leads in undertaking elements of the statutory role.
- To take responsibility for providing advice, guidance, mentoring and targeted interventions on an individual and/or group basis to Havering’s children looked after and care leavers
- To positively contribute and develop service delivery across Council departments and the multi-agency network of professionals.
- To supervise and mentor Young Person Advisors and to transfer knowledge and best practice into business as usual.
- To Supervise and mentor non-social work qualified Pathway Co-Ordinators
- To lead in group work that supports transitions to adulthood
Participate in Deep-Dives to help self-evaluation of the service
You will have experience or a qualification in one of the below fields or a similar field:
-General and Sexual Health
-Housing and Homelessness
-Alcohol and Substance Misuse
-Education, Employment, and Training
-Immigration and UASC
You must have a valid DBS. Ideally, you will have successfully completed the foundation year in systemic practice.
You will have good knowledge of how Children’s Services operate, both in education and social care context.
You will have excellent communication skills, which are fundamental to the role, as you will liaise and work with a range of professionals across the Council and region.
Essential to the role are excellent organisational skills and the creativity required to co-ordinate services across the children and adult services to provide a continuity of care. You will demonstrate the ability to work in partnership and collaborate with young people
This role will be responsible for working closely with the multi-agency network of professionals to passport young people to services based on need as opposed to eligibility.
This role will manage a caseload and co-produce care plans and services to improve outcomes for care leavers. This role will work as part of a team and develop an innovative model of outreach and engagement with young people.
