Care Co-ordinator
Could you work alongside the existing Care Co-ordinator one day in the week and provide outstanding leadership to the domiciliary care team at weekends?
Due to the success of Cheviot Gardens in West Norwood in providing increased care hours and the success of Conrad Court in Lewisham in arranging for extra management cover, we are now looking to provide enhanced management at weekends.
We are looking for two part-time Care Co-ordinators to support our customers, including many vulnerable adults, and to achieve positive outcomes, including improving health and wellbeing, social interaction and tenancy sustainment.
As part of the management team, you will be putting our customers at the very centre of the personalised services we deliver. You will be trained in effective needs assessment methods that focus on how our customers want to be living their lives.
You will write comprehensive support plans, that ensure every aspect of a customer’s life is promoted so they are safe, secure and are able to maintain a positive, happy and dignified way of living.
Therefore, you will be quick to build relationships with customers, their relatives/carers as well as partner agencies, and be competent at expressing yourself in writing.
- Cheviot Gardens is looking for a Care Coordinator to work 20 hours per week on Friday, Saturday and Sunday.
- Conrad Court is looking for a Care Coordinator to work 18.5 hours per week on Saturday, Sunday and Monday.
