Social Worker
The role
We are looking for an enthusiastic, committed and creative social worker to join Southwark's Move on Support Team within the Mental Health Adult Social Care Division, for a 1 year fixed term position.
The team sets out to explore real opportunities for adults placed in various mental health accommodation (supported, residential and nursing settings). Via strength-based practice, our focus is to maximise a person's independence, to offer choices, to promote wellbeing and to meet our duties under the Care Act. We have the scope to develop creative and innovative approaches to ensure service users are at the centre of the support they receive.
We are a team of social workers and occupational therapists working alongside our health colleagues and external providers. Working in collaboration, as part of the assessment and review process, you will ensure that current support plans are personalised, recover focused and promote independence. Part of the role is to establish if service users are in the most appropriate placement to meet their needs or if an alternative to this should be sought.
Southwark Council is committed to continuous learning. We provide our workforce with regular supervision, reflective practice, specialist training and continuing professional development. The post offers the opportunity to practice your core social worker skills, by providing a range of detailed assessments and interventions to ensure that all placements/care is appropriate and meets the current needs of service users. The post holder may need to travel outside of London to visit placements.
