Senior Practitioner
As a Senior Practitioner within our Adult Services Integrated Hospital Discharge Service, you will be key to providing support to practitioners at all levels of their career development, utilising your supervisory skills and deputising for the Team Manager in helping support the team to achieve the very best outcomes for our residents, and to meet challenging targets for our residents who find themselves in hospital in Hackney or elsewhere.
You will have a sound knowledge of the key principles of the Care Act, the Mental Capacity Act, Safeguarding and Delayed Transfers of Care, as well as all other relevant legislation relating to the provision of services for adults with long term conditions. You will have a commitment to your own and your supervisee’s social work education and be ready to help develop our Social Work offer through the academy approach.
A commitment to working in an integrated manner with health colleagues both in the team and across the hospital is essential, as well as an understanding of the value of working with the wider multi-disciplinary team such as the Police, Housing, Education and Children’s Services.
We are looking for someone who can make a positive contribution to the work of Adult Services as a whole, and by offering the provision of professional supervision and management of duty systems, allocation of casework and leading on safeguarding as well as managing a caseload.
You will be an enthusiastic proponent of promoting every individual service users independence wherever possible, with a sound understanding of personalisation including use of Direct Payments.
You will need to be a qualified Social Worker and be registered with HCPC. You will also need a good level of post qualification experience within statutory Adult Services.
This role is an opportunity for those who have used their expert knowledge to help guide and assist other staff members, through mentoring, training and offering advice.
