To support the delivery of an integrated social care service to an allocated caseload within the agreed performance management, quality assurance and budgetary framework and that the highest standards of service are achieved.
To act as a member of a team providing an effective, efficient and personalised assessment, support planning, brokerage and review service to adult service users, carers and their families in order to deliver the key strategic aims of the organisation and to ensure that the citizens of Newham have access to high quality services which safeguard and promote their welfare.
To ensure that the they follow the principle of strength based approach is applied within their day-to-day practice so that all people are fully supported to have the best possible personalised opportunities and are supported to exercise choice and control to meet the outcomes they define.
Newham Adult Social Care services provides support to adults and carers, who may, because of the effects of ill health or disability, need support to live safely and independently. The Person
We are looking for 2 Social Care Officers who have and want to further develop their skills with case work under the Care Act like promoting independence and Direct Payments, carrying out reviews and working with customers and carers in the community and building good links with community agencies in the new neighbourhood structure in the Service (Adults). We are looking for Social Care Officers who have good assessment and care planning skills and passionate about delivering results and outcomes.
