What You Will Do

  • Care Act assessments – you will carry out Care Act assessments  with Adults who have been discharged from hospital 
  • Support plans – you will complete and review care and support plans for Adults who have been discharged from hospital 
  • Mental Capacity assessments- you will complete MCA with Adults either in hospital or in the community 
  • Work creatively with Adults, their families and carers- you will using a strengths based approach and evidence based practice with a good understanding of risk assessments and positive risk taking make the best use of available resources to enable people to have better lives and remain as independent as possible 
  • Arranging hospital discharges – in conjunction with health colleagues support Adults to remain as independent as possible, maintain their wellbeing, manage risk and participate in the life of their community
  • You will work with those that self neglect or hoard 

What You Need

  • Social Work qualification
  • SWE registration
  • 2 years post qualifying experience
  • Excellent communication skills
  • Empathy
  • Critical thinking skills
  • Self awareness
  • Cultural competence

This is a full time role for 36 hours a week.  Due to the nature of the work staff work weekends on a rota basis with time off in lieu and enhanced pay for hours worked at weekends.
