Not afraid of a challenge? We need a committed knowledgeable Health and Wellbeing Assessor to help to facilitate the work of the LIFE service enabling people to gain independence at home and achieve their full potential.
At this difficult time the Council Leadership has made clear their support for our statutory functions, including Adult Social Care. We know where our priorities are.
We have an established and stable base of Team Managers and social workers to work with you and support you, our vacancy rates are low - and we rarely use agency staff, preferring to concentrate our efforts on looking after our permanent staff. This includes having a dedicated Training officer supporting adult social care, and a Deputy Principal Social Worker to support staff welfare and practice. We have also recently upgraded our electronic recording system.
Health and Well Being Assessor – LIFE Service
You will work as a trusted assessor in the LIFE team providing skilled support to Social Workers, Occupational Therapists, Physiotherapists, Intermediate Care Nurses, and will undertake duties as allocated to you by social workers and a qualified member of the Nursing and Therapy staff.
The role tangibly contributes to providing support, empowerment, independent living skills and opportunities that promote people reaching their full potential as individuals.
You will be based in the Community and will be expected to work autonomously in a variety of settings including in the person’s home. Health and Wellbeing Assessors at Grade 8 will provide some supervisory support to staff at a lower level.
