These posts will be within the Leyton Early Help Delivery Team.
The post holders will deliver integrated and comprehensive early intervention services to children and young people and their families with the aim of supporting them to achieve positive outcomes, increase their resilience and improve family functioning.
The post holders will manage a caseload of families, conducting in depth assessments to identify needs, utilise the Team Around the Family process to effectively action plan with the family and other professionals.
The post holders will ensure that families and or individuals are met with in according with Early Help standards to ensure that there is a timely review of the progress of the family.
The post holders will form and maintain good links with partners and coordinate services and, where applicable, source resources to aid the delivery of services to the families. This will include working with the Children & Families Centre.

The design and delivery of effective whole family interventions as the Lead Professional with a caseload of vulnerable families many experiencing multiple needs.
Ensuring continuity and consistency in delivery of early help assessments using the Our Family Journey tool with families, including Family Outcomes Planning and review through home visits, parenting support, chairing Team Around the Family meetings and professionals’ meetings.
Carry out evidence based -based practice that builds skills, relationships and resilience, helping families to help themselves and each other.
Promote effective integrated working across the children’s services system: develop effective working relationships with families and communities, within Families First, within the Early Help Division, and with professional partners from a range of agencies and sectors. Carry out multi-disciplinary and co-located team working and coordinate multi-agency Family Outcomes Plans that build skills and relationships and improves outcomes for families.
