Wanstead and Woodford HASS as part of London Borough of Redbridge and NELFT NHS Foundation Trust are looking for a motivated, skilled, experienced person to work as an experienced Social Worker as part of our multi-disciplinary team.
About Community Health and Social Care services in Redbridge
Community Health and Social Care teams consist of district nurses, social workers, rehabilitation therapists, occupational therapists, MH workers, LD workers. We work in partnership with other health and social care teams such as specialist nursing or children services.
This integrated way of working allows us to deliver high quality person centred services that meet the needs of the local residents. Our services are innovative and rewarding for both clients and the workers.
As an experienced Social Worker, you will deliver person centred holistic support planning to maintain independence by promoting health and wellbeing using a People Matter Approach through 3 conversation models. Undertaking a Care-co-ordination role in maintain older people’s wellbeing. You will have experience working with a wide range of people in a health and social care setting and be ready to progress to a Level 3 now or in the near future with our career development support.
